Макролидный иммунодепрессант, ингибирующий кальциневрин, который используется для предотвращения отторжения трансплантированных органов (почек, печени, сердца...) и для лечения атопической экземы, псориаза и других аутоиммунных заболеваний (например, артрита, волчанки, воспалительного заболевания кишечника...) . Пути введения: пероральный для системных целей и местный при дерматологических заболеваниях.
Высокая молекулярная масса и высокая способность связывания с белками объясняют, что он был обнаружен в клинически незначительных количествах в молозиве и грудном молоке (Jain 1997, 2003 French, Gardiner 2006, 2013 Zheng, Bramham 2013).
Никаких проблем с краткосрочными или долгосрочными исходами развития или иммунологическими показателями не наблюдалось у более чем 150 младенцев, матери которых лечились (French 2003, Gouraud 2012, Thiagarajan 2013, Constantinescu 2014, Izumi 2014), за исключением легкого и самокупирующегося случая транзиторного тромбоцитоз (Gouraud 2012).
Уровни препарата в плазме у этих младенцев были либо неопределяемыми, либо очень низкими (Gomez-Lobo 2012, Gouraud 2012, Bramham 2013, Izumi 2014).
Низкая пероральная биодоступность, которая может дополнительно снижаться при приеме пищи, особенно если это жирная пища, будет препятствовать прохождению лекарства из проглоченного молока в плазму младенцев.
Младенцы, подвергшиеся воздействию ингибиторов кальциневрина во время беременности, могут иметь иммунологическую недостаточность в течение первого года жизни (Grimer 2007), поэтому очень важно избегать прекращения грудного вскармливания без веской причины.
Несколько медицинских обществ и консенсус экспертов сочли безопасным использование такролимуса при грудном вскармливании (Østensen 2006, van der Houde 2010, Thiagarajan 2013, Armenti 2013, Constantinescu 2014, Schulze 2014, Durst 2015, Götestam-Skorpen 2016, Flint 2016).
